HORIZONS - CASE MANAGER - CRISIS INTERVENTION SERVICES Category: Allied Health Facility: Horizons Mental Health Center Department: CRISIS MANAGEMENT Schedule: Full Time - more than 72 hrs a p.p. Shift: Day Shift Hours: 8-5 varies Job Details: Description: The Crisis Intervention Case Manager (CI -C) is a member of the Community Based Service Department. This position works as a member of the Crisis Intervention team, which is led by the Crisis Intervention QMHP (CI-Q). The team works closely with all Horizons clinical and community staff to provide services at a higher level of intensity or frequency with the goal of community tenure by the consumer. Requirements: Education: Bachelors degree in a Behavioral Health field or Special Education. Experience: Three years experience in behavioral health preferred as is experience in providing crisis intervention services. Training: Within 30 days of employment the employee must have completed CPST Case Management on-line training, KHS on-line training. Within 60 days of employment, the employee must have completed Adult Case Management training. Position Knowledge: Familiarity with community mental health services in general and services within Horizons Mental Health Center preferred.
